This role provides high‑quality health services through clinical care, case management, health education, emergency response, and community outreach within Cuyahoga County. PHNs deliver assessments, screenings, vaccinations, counseling, and referrals in clinic, school, and home settings while maintaining accurate documentation and compliance with public health standards.
