Date: Apr 20, 2026 Location: CLEVELAND, TX Position Title: Public Health Nurse I Agency: Dept of State Health Services Department: Region 6/5 Salary Range: $5,521.16 - $6,779.25 per month Closing Date: 10/19/2026 Overview Functions under the Community Health Services Manager with direct supervision by the 6/5S Nurse Manager III under the general direction of the Regional Medical Director. Utilizes the nursing process in assessing, planning, implementing, coordinating, delivering, and evaluating population‑based, specialized public health nursing services. Travel required throughout the seven‑county region, primarily focusing on Liberty County. Services are provided to individuals, families, groups, and communities with health needs ranging from simple to complex, focusing on optimum wellness and disease prevention. Collaborates with other regional programs on community health and communicable disease initiatives. Participates in coordinating day‑to‑day operations of clinic services and provides planning for the assigned community health services team. Requires extensive individual initiative in instituting, monitoring, and evaluating projects, practice in an autonomous setting, provide guidance to other nurses, technical assistance to local providers, preventive and limited treatment services, and time‑management skills to prioritize daily duties. Essential Job Functions Population‑Based Services (45%): Provides essential public health nursing services as part of the family health services team in assigned communities. Establishes and maintains community partnerships; participates in assessments, identifies needs, plans and implements interventions using evidence‑based strategies. Monitors progress, evaluates results, modifies plans, implements community education programs, assists with preparedness activities, participates in infrastructure building, provides consultation and training to providers and community, represents DSHS on coalitions and advisory boards. Applies epidemiological principles and methods; required to take and certify Child Passenger Safety training. Direct Clinical Services (30%): Provides specialized and comprehensive public health nursing services in the clinic, home, and community, including immunizations, TB, STD testing and treatment. Uses nursing process for assessment, planning, implementation, and evaluation. Provides individualized care per policies, procedures, and orders. Develops individualized care plans, lab testing, counseling, field visits, medication administration, education, case management, referrals, follow‑up; responsible for accurate documentation, vaccine handling, phlebotomy skills required. Coordination & Leadership (20%): Participates in evaluation of nursing services, training/orientation of staff, interpreting program operations, writing and monitoring annual clinic/team goals. Coordinates schedules, delegates work, manages supplies. Conducts quality assurance monitoring, reviews records and operations, makes improvement recommendations, prepares reports, participates in interdisciplinary teams, may lead day‑to‑day clinic operations and staff oversight. Provides guidance, collects data, and gives input to supervisor. Other duties (5%): Participates in disaster response or continuity of operations (COOP) activation, support for disaster response, community preparedness activities; may require alternate shift patterns or location changes. Licensure, Certification, or Registration Required Must be licensed as a Registered Nurse by the State of Texas or a state that recognizes reciprocity through the Nurse License Compact. Must maintain current American Heart Association Basic Life Support certification. Knowledge, Skills, and Abilities Required Knowledge of professional nursing theory, nursing process, legal framework of nursing (Texas State Board of Nurse Examiners rules, Nurse Practice Act, standing delegation orders), pharmacy and CLIA regulations, public health principles, epidemiology, disease prevention, health promotion, teaching/learning theories, cultural, social, economic factors, community resources, quality assurance principles. Skills in assessment of clients with complex health problems; direct nursing care; phlebotomy; vaccine administration; basic computer use (data input, word processing, spreadsheet, e‑mail, internet); problem solving, prioritizing, planning, executing work; effective written and verbal communication; teamwork; educational presentation development; building collaborative relationships; traveling; meeting immunization requirements; maintaining confidentiality and HIPAA compliance; working evenings and weekends occasionally; scheduling and time management; Spanish preferred. Must be able to administer immunizations per standing delegation orders, operate state‑owned or personal vehicle for state business, meet driving‑record requirements, maintain acceptable record, comply with DSHS immunization policy. Initial Selection Criteria Must be a registered RN in Texas or a state covered by the Nurse Licensure Compact; BSN preferred; minimum of 3 years of experience performing public health population‑based and clinical activities. Additional Information Provide own transportation; must meet driving policy; daytime and occasional late‑afternoon travel to assigned rural counties; mileage reimbursed per state policy; serve as essential personnel in disaster response activities within regional jurisdiction, including alternate work hours.
