This role coordinates the RAI process, collaborates with an interdisciplinary team, and transforms clinical information into individualized care plans that promote dignity, independence, and community inclusion. The MDS RN ensures compliance with CMS and state regulations, supports quality improvement, and mentors staff on documentation and assessment best practices within our faith-inspired, mission-driven culture.
